ENTERING STUDENT-ATHLETES
Welcome to the Towson University Tiger Athletic family. Following are the necessary forms, sheets and documents required for medical clearance. Each must be completed and returned to Towson University Sports Medicine by every entering student-athlete. Incomplete documents will be returned to you and will delay the process. Student-athletes who are NOT medically cleared, are not permitted to work-out, practice, compete, condition, weight-train, receive equipment or athletic issued clothing until all required documents have been received, evaluated and approved. Please fill out completely, leave no areas blank.

Introduction to Medical Clearance
Schedule an appointment with a physician for a pre-participation physical examination. A current examination is required and is defined as being performed within 90 days of being received by the Sports Medicine staff. You must download and print the following forms for you and your physician to complete.

Please download, print and complete the following forms: (Please note that you must copy, paste or otherwise attach a COPY OF THE FRONT & BACK of your medical and prescription insurance card in the provided space)

ALL MEDICAL CLEARANCE DOCUMENTS ARE TO BE RECEIVED NO LATER THAN THE FOLLOWING:
If your 1st practice date is prior to the first day of classes: June 30, 2008
All others: July 21, 2008
