RETURNING STUDENT-ATHLETES
We look forward to your return to the Tiger Athletic family for another exciting year in the pursuit and achievement of athletic excellence. Following, are the documents required to complete the annual medical clearance process. Incomplete forms will be returned to you and will delay the clearance process.
Student-athletes who are NOT medically cleared, are not permitted to work-out, practice, compete, condition, weight-train, receive equipment or athletic issued clothing until all required documents have been received, evaluated and approved. Please fill out completely, leave no areas blank.

REMINDER: If you were injured or, received medical treatment for any medical condition or injury during the summer, you must submit to Sports Medicine, ALL documentation related to that injury or condition. The documentation should include, but not limited to, medical and / or surgical notes, dictations, lab or radiographic studies and reports, therapy notes as well as physician clearance to participate in your specific intercollegiate sport. If there are limitations noted, you are responsible for securing clearance from the treating physician, prior to being evaluated and cleared by the University's Team Physician.
All medical clearance documents are to be received no later than:
If your 1st practice date is prior to the first day of classes: June 30, 2008
All others: July 21, 2008
